Where the data comes from
Every record on this site comes from the public National Provider Identifier (NPI) records that the Centers for Medicare & Medicaid Services (CMS) publishes through NPPES. CMS is the authoritative source for NPI records.
How we keep it current
CMS updates NPPES regularly. We update this lookup from the latest CMS records and show the most recent update date on the data freshness page. A provider may have updated their record with CMS more recently than our latest update.
How we present it
- We tidy names and addresses for readability without changing their meaning.
- We standardize dates and show whether a record is active or deactivated in NPPES.
- We add short, plain-English labels for specialties, based on the standard NUCC Health Care Provider Taxonomy code set.
What we deliberately leave out
This is an NPI lookup, and nothing more. We do not include — and do not infer — a license, credentials, board certification, clinical quality, sanctions, malpractice history, payments, insurance networks, reviews, or ratings. Public NPI records cannot support those judgments, so we do not make them.
Confirming important details
NPPES is administrative data. For anything that matters, confirm details with CMS NPPES, the relevant state licensing board, the provider's office, or your insurer. See our disclaimer for the full limits of this information.
