1. Acceptance of these Terms
These Terms and Conditions (the “Terms”) govern your access to and use of Dr. NPI Search and its related pages, content, and features (the “Site”). The Site is operated by PGBDIC LLC, an Illinois limited liability company (“PGBDIC LLC,” “we,” “us,” or “our”). By accessing or using the Site, you agree to be bound by these Terms and by our Privacy Policy. If you do not agree to these Terms, you must not access or use the Site. If you use the Site on behalf of an organization, you represent that you have authority to bind that organization to these Terms.
2. What the Site is — and what it is not
The Site is an informational, public-data lookup tool. It provides access to public National Provider Identifier (NPI) record information derived from the U.S. Centers for Medicare & Medicaid Services (CMS) National Plan and Provider Enumeration System (NPPES) Data Dissemination files and related public administrative data. We organize, normalize, index, cache, and display this public data for usability; we do not create the underlying records.
PGBDIC LLC and the Site are independent and are not affiliated with, endorsed by, sponsored by, or operated by CMS, the U.S. Department of Health and Human Services (HHS), NPPES, any licensing board, any healthcare provider or organization, any insurer, or any government agency. PGBDIC LLC is not a healthcare provider, credentialing body, licensing board, insurer, consumer reporting agency, medical advisor, or government agency.
3. NPI limitation — important
An NPI is a public administrative identifier only. The presence of an NPI record, or any field within it, does not confirm any of the following: that a person or organization holds a license or is licensed; licensure status or good standing; credentialing; board certification; clinical quality, safety, competence, or reputation; participation in any insurance network; employment; current practice status; the accuracy of any name, address, or other field; whether a provider is accepting new patients; eligibility for payment; or the legal right or authority to provide healthcare services.
You must independently verify a provider’s identity, licensure, credentials, and suitability for your purpose with the appropriate state licensing boards, healthcare organizations, insurers, CMS/NPPES, and other authoritative sources before relying on any information.
4. No professional advice
The Site does not provide medical, legal, credentialing, compliance, insurance, employment, or other professional advice, and using the Site does not create any professional relationship. Do not use the Site for emergency medical decisions. The Site must not be used as the sole basis for any hiring, credentialing, contracting, payment, insurance, referral, clinical, employment, or patient-care decision. Always consult qualified professionals and authoritative sources.
5. Your responsibility; no reliance
You are solely responsible for:
- independently verifying all information before relying on it;
- complying with all applicable laws and regulations;
- using the Site only for lawful purposes; and
- determining whether any provider, organization, or record is appropriate for your intended use.
You agree not to rely on the Site as a sole or authoritative source, and you assume all risk arising from any reliance on information obtained through the Site.
6. Data source and accuracy limitations
Information on the Site is derived from public source data and may be incomplete, outdated, delayed, inaccurate, misspelled, duplicated, deactivated, or reported differently in the underlying source systems. Source data may change without notice. We may transform, normalize, index, cache, summarize, or display the public data for usability, which may introduce differences from the source records.
PGBDIC LLC does not warrant or guarantee the completeness, accuracy, timeliness, availability, or fitness for any particular use of any information on the Site. The Site may update data periodically but does not guarantee any specific refresh cadence except as stated on the data-freshness page. For authoritative confirmation, consult the official CMS/NPPES records and the relevant licensing boards.
7. Deactivated records
A “deactivated” NPI means the NPI record carries a deactivation status or date in the source data. A deactivation status alone does not explain why an NPI was deactivated and is not a finding of misconduct, discipline, fraud, malpractice, sanction, complaint, or any quality or competence determination about any person or organization. You must not infer misconduct, wrongdoing, or any assessment of a provider from a deactivation status.
8. No ratings, rankings, or recommendations
The Site does not recommend, endorse, rank, rate, review, credential, approve, or certify any provider or organization, and it does not assess the quality, safety, or competence of any provider or organization. Search results, related links, directory listings, taxonomy pages, and provider pages are not recommendations or endorsements. Any ordering reflects data availability, search relevance, public-data structure, or technical ranking only — not quality, endorsement, or suitability.
9. Permitted use
Subject to these Terms, PGBDIC LLC grants you a limited, revocable, non-exclusive, non-transferable license to access and use the Site for reasonable personal, professional, and internal business public-data lookup, provided you comply with these Terms and all applicable laws.
10. Prohibited use
You agree that you will not, and will not permit any third party to:
- use the Site for any unlawful purpose or in violation of any applicable law;
- use the Site to harass, defame, threaten, discriminate against, or otherwise harm any provider, organization, or person;
- use the Site to make unsupported or misleading claims about any provider or organization;
- scrape, crawl, spider, harvest, bulk-extract, resell, redistribute, or create derivative databases from the Site or its content without our prior written permission;
- bypass, disable, or interfere with any security, rate-limiting, or technical control;
- interfere with or disrupt the Site’s security, integrity, availability, or performance;
- reverse engineer, decompile, or disassemble any part of the Site;
- misrepresent your affiliation with PGBDIC LLC or the Site;
- use the Site or its data for any purpose governed by the Fair Credit Reporting Act (FCRA) or any similar consumer-reporting law (see Section 11);
- use the Site as a sole basis for any credentialing, employment, insurance, payment, contracting, clinical, or patient-care decision; or
- use the Site to build provider scores, sanction lists, complaint lists, risk flags, or similar assessments of providers or organizations.
11. No FCRA or consumer-reporting use
PGBDIC LLC is not a consumer reporting agency, and the Site does not provide consumer reports or any “consumer report” as defined by the FCRA. You may not use the Site or its information, in whole or in part, as a factor in establishing any individual’s eligibility for employment, credit, insurance, housing, credentialing, or any other purpose regulated by the FCRA or any similar federal, state, or local law. By using the Site, you certify that you will not use it for any such purpose.
12. Accounts, tools, APIs, and future services
PGBDIC LLC may offer accounts, tools, APIs, monitoring, data downloads, or other features in the future, which may be subject to additional terms. We may add, limit, suspend, condition, or terminate access to any feature at any time, and we may block or restrict access that we determine to be abusive, automated, excessive, insecure, or otherwise contrary to these Terms.
13. Intellectual property
The Site’s design, compilation, organization, code, user interface, original written content, branding, and other original materials are owned by PGBDIC LLC or its licensors and are protected by applicable laws. Public source data remains subject to its source status and any applicable terms. Except for the limited license in Section 9, no right, title, or interest in the Site is transferred to you.
14. Third-party data and links
The Site may reference third-party public datasets, government resources, or external links and websites. PGBDIC LLC does not control and is not responsible for the content, availability, accuracy, or practices of any third-party resource, and inclusion of any link is not an endorsement.
15. Disclaimer of warranties
THE SITE AND ALL INFORMATION, CONTENT, AND FEATURES ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND. TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE PROTECTED PARTIES DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, NON-INFRINGEMENT, ACCURACY, COMPLETENESS, AVAILABILITY, AND UNINTERRUPTED OR ERROR-FREE OPERATION. No information on the Site creates any warranty or professional relationship.
16. Limitation of liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT WILL PGBDIC LLC AND ITS OWNERS, MEMBERS, MANAGERS, OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, AGENTS, AFFILIATES, SUCCESSORS, ASSIGNS, LICENSORS, SERVICE PROVIDERS, AND DATA SUPPLIERS (COLLECTIVELY, THE “PROTECTED PARTIES”)BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR FOR ANY LOST PROFITS, LOST REVENUE, LOST DATA, BUSINESS INTERRUPTION, REPUTATIONAL HARM, OR RELIANCE DAMAGES, OR FOR ANY HEALTHCARE, CREDENTIALING, EMPLOYMENT, INSURANCE, CONTRACTING, PAYMENT, OR CLINICAL DAMAGES, ARISING OUT OF OR RELATING TO THE SITE OR THESE TERMS, REGARDLESS OF THE THEORY OF LIABILITY (CONTRACT, TORT, NEGLIGENCE, STRICT LIABILITY, STATUTE, EQUITY, OR OTHERWISE) AND EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE TOTAL AGGREGATE LIABILITY OF THE PROTECTED PARTIES FOR ALL CLAIMS RELATING TO THE SITE OR THESE TERMS WILL NOT EXCEED THE GREATER OF (A) THE TOTAL AMOUNTS YOU PAID TO PGBDIC LLC FOR THE SITE IN THE THREE (3) MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM, OR (B) ONE HUNDRED U.S. DOLLARS (US$100). Some jurisdictions do not allow certain limitations, so some of the above may not apply to you; in that case, the limitations apply to the maximum extent permitted by law.
17. Indemnification
You agree to defend, indemnify, and hold harmless PGBDIC LLC and its owners, members, managers, officers, directors, employees, contractors, agents, affiliates, successors, assigns, licensors, service providers, and data suppliers (collectively, the “Protected Parties”) from and against any and all claims, demands, actions, investigations, liabilities, damages, losses, settlements, judgments, fines, penalties, costs, and expenses (including reasonable attorneys’ fees) arising out of or relating to: (a) your access to or use of the Site; (b) your reliance on any information from the Site; (c) your use or misuse of any NPI, provider, organization, taxonomy, address, endpoint, or deactivation data; (d) your violation of these Terms; (e) your violation of any law or any third-party right; (f) your use of the Site for any credentialing, employment, contracting, insurance, payment, patient-care, clinical, compliance, or eligibility decision; (g) any communication, publication, report, product, or service you create based on the Site or its data; (h) any scraping, bulk extraction, automated access, resale, republication, or derivative database; and (i) any claim by a provider, organization, patient, employer, insurer, regulator, or other third party arising from your use or misuse of the Site.
PGBDIC LLC may, at its option, assume the exclusive defense and control of any matter subject to indemnification, select counsel, and approve any settlement, and you agree to cooperate. You may not settle any such matter in a manner that imposes any obligation or admission on any Protected Party without PGBDIC LLC’s prior written consent.
18. Release
To the maximum extent permitted by law, you release the Protected Parties from any and all claims, demands, and damages arising out of or relating to your reliance on public data, errors or omissions, source-data limitations, disputes with providers or organizations, decisions made by you or any third party, or your use or misuse of the Site.
19. Corrections and removal requests
Because the Site reflects public NPPES records, the authoritative way to correct the underlying record is to update the official NPPES record with CMS/NPPES (see how to update an NPPES record). If you believe a record is displayed incorrectly on the Site, you may contact us and we may, in our sole discretion, review and address a Site-display issue. PGBDIC LLC is not obligated to remove or alter accurate public records except as required by law.
20. Privacy
Your use of the Site is also governed by our Privacy Policy, which is incorporated into these Terms by reference.
21. Availability and changes
We may change, suspend, degrade, limit, or discontinue the Site or any feature at any time, with or without notice. Data may be delayed, unavailable, or cached, and features may be added or removed. We may update these Terms from time to time; the “Last updated” date above reflects the most recent version, and your continued use of the Site after changes take effect constitutes acceptance of the updated Terms.
22. Governing law and venue
These Terms are governed by the laws of the State of Illinois, without regard to its conflict-of-law principles. Subject to Section 23, you agree that any dispute will be brought exclusively in the state or federal courts located in Cook County, Illinois, and you consent to the personal jurisdiction of those courts and waive any objection to venue there.
23. Dispute resolution; arbitration and class-action waiver
Please read this Section carefully. To the maximum extent permitted by law, any dispute, claim, or controversy arising out of or relating to the Site or these Terms will be resolved by binding individual arbitration administered under applicable arbitration rules, seated in Cook County, Illinois, rather than in court, except that either party may bring an individual claim in small-claims court, and either party may seek injunctive or equitable relief in court to protect intellectual property or Site security.
To the maximum extent permitted by law, you and PGBDIC LLC each waive any right to a jury trial and agree that claims may be brought only in an individual capacity, and not as a plaintiff or class member in any class, collective, or representative proceeding. You may opt out of this arbitration agreement by sending written notice to hello@verifydoctornpi.com within thirty (30) days of first accepting these Terms; opting out does not affect the other provisions of these Terms.
24. Termination
We may block, suspend, condition, or terminate your access to the Site at any time, with or without notice, including for any violation of these Terms or for scraping, abuse, security risk, or legal-risk reasons. Sections that by their nature should survive termination — including the disclaimers, limitation of liability, indemnification, release, intellectual property, governing law, and dispute-resolution provisions — will survive.
25. General
If any provision of these Terms is held unenforceable, that provision will be limited or severed to the minimum extent necessary, and the remaining provisions will remain in full force. Our failure to enforce any provision is not a waiver. You may not assign these Terms without our prior written consent; PGBDIC LLC may freely assign these Terms. These Terms, together with the Privacy Policy, are the entire agreement between you and PGBDIC LLCregarding the Site and supersede any prior agreements on that subject. Headings are for convenience only and do not affect interpretation.
